Main Duties of the Job
Days worked Monday to Friday with weekends on a rota basis.
Provide high‑quality, person‑centred care for individuals facing life‑limiting illnesses, extending beyond direct patient care to include education, leadership and advocacy.
Manage your own caseload and deliver expert care tailored to each service user’s needs, wishes and culture.
Collect, collate, evaluate and report information, maintaining accurate user records.
Job Responsibilities
Support, assess and advocate for service users and carers dealing with complex, distressing and life‑limiting illnesses.
Ensure patients and families experience dignity, comfort and peace during challenging times, enhancing overall quality of palliative care.
Collaborate with the healthcare team to deliver the best possible outcomes.
Person Specification
* Registered with the NMC and possess a Specialist nursing qualification with experience in complex care.
* Significant experience in Palliative and End‑of‑Life Care and evidence of additional, role‑specific qualifications and professional development.
* Ability to support adjustments for diverse patient needs.
* Skilled in accessible communication with patients and families.
* Commitment to inclusion and patient advocacy.
* Advanced communication training (e.g., Sage & Thyme).
* Experience with symptom management in complex needs.
* Willingness to engage in induction, supernumerary time and full training/competency assessment.
* Desire to develop skills in specialist palliative care with planned career progression through education programmes.

Salary
£36,058.02 to £46,250.96 per annum, pro rata.
