Role
The Nurse Practitioner provides care appropriate to the patient population served, including routine health maintenance, management of chronic conditions, and management of acute illnesses. This is a full-time position, with hours that may include evening shifts and Saturday rotations based on clinic needs.
Responsibilities
* Serve as a primary care provider for a panel of patients.
* Manage care of primary care patients, including outpatient visits, communication with patients, inpatient care, referrals, patient education, risk assessment, disease prevention, and evidence-based medicine.
* Perform routine health maintenance, including history and physicals, screenings, health promotion, and follow-up care.
* Manage common chronic illnesses, providing or arranging appropriate patient education.
* Participate in an on-call system providing 24-hour access to a provider.
* Participate in evening, Saturday, and holiday clinic rotations.
* Attend staff meetings, trainings, and quality improvement sessions.
* Support managed care plans, including patient education and referral management.
* Maintain and update Electronic Medical Records (EMR) accurately and timely.
Requirements
* Completed accredited Nurse Practitioner program in Family Practice.
* Current MA NP License, Board Certification, DEA, MCSR, and BLS required.
* Computer literacy (MS Office, Windows), experience with EMR (EPIC/OCHIN preferred).
* 3-5 years of NP experience required; bilingual skills preferred.
* Experience with community health center populations is a plus.
