Could you help West Suffolk Hospital to deliver the best quality and safest care to our local community?

An exciting opportunity for a band 6 Junior Sister, Community Nurse, has arisen in the Bury Town Integrated Neighbourhood Team. The team are based in Bury St Edmunds a historic market town surrounded by picturesque villages.

The successful applicant will join a dynamic, friendly team of community nurses, community matrons, therapists. You will be providing nursing services to patients in the community and have the chance to work in an autonomous manner with the support of the wider multi disciplinary team within the hospital and across the West Suffolk Alliance.

Within your role you will be able to provide clinical and supervisory leadership to staff and students within the team and support the management of the nursing staff by taking responsibility, in partnership with the District Nurse, for the day-to-day co-ordinating, planning and delivery of the nursing services within the Community Healthcare Team. You will be supporting the management of patients with multiple and complex needs, using evidence based practise to assess, plan, implement and evaluate interventions to improve patient health.


Main duties of the job

You will be using a wide range of nursing skills and will need to be confident in your assessment skills and in your ability to provide safe, effective treatments whilst working in a community setting.

Experience in leadership as well as clinical experience in palliative care, wound management, catheter management, cannulation, IVs and a range of point of care testing techniques will be an advantage. You will have the chance to be fully involved in working with the MDT and integrating with Adult Social Care, hospice and hospital staff and will need to be committed to ongoing learning and development in the team.

This will be the perfect opportunity if you would like a role where you can use a broad range of nursing skills and develop your leadership and management skills. We deliver nursing care to patients in their own homes and on the virtual ward.

You will be able to develop your role in accordance with areas of clinical interest and to progress in your career, with training and education available to support this. There are opportunities to be involved in quality improvement initiatives and ongoing learning and development in the team.

We operate a 7 day service within core hours 8am to 6pm


Person Specification


Education & Qualification

* Current NMC registration or working towards.
* To have or be willing to work towards one or a combination of the following: oLeadership training.
* To have or be willing to work towards one or a combination of the following: oLevel 3 degree Clinical module on long term conditions such as Leg Ulcer Management, Diabetes care or End of Life Care.
* To have or be willing to work towards one or a combination of the following: oCare Lead / Case Management /Health coaching training.
* Evidence of continuing professional development recent and relevant to the post.
* Demonstrate knowledge, understanding and use of HR management policies including Capability, Sickness management, and Disciplinary.


Experience & Knowledge

* Evidence of working within the community nursing setting.
* Experience of supervision of staff, including PDR.
* Evidence of high level of ability to reflect on and critically appraise own practice and performance.
* Experience of co-ordinating and delegating work and reviewing others performance.
* Experience and knowledge of autonomous and independent decision making in home environments, which are often complex and unpredictable.
* Enhanced awareness of mental health conditions and issues.
* Comprehensive holistic assessment skills in the patient's home environment.
* Evidence of formal clinical supervision.
* Ability to give constructive feedback and praise


Skills & Abilities

* High level of nursing skills and knowledge for managing and supporting the care of people in their own home environment.
* High level of ability to work under pressure and set priorities to achieve service objectives.
* High level of clinical reasoning skills and credibility as a nursing leader.
* Knowledge of additional specialist areas through in-house training and short courses.
* Effective communication skills with people on complex matters, issues, ideas, in complex situations.
* Ability to work alone and give professional supervision and guidance to junior staff.
* Ability to act on own initiative and manage any risks.
* Good observational and reporting skills
* Able to give examples of working flexibly to accommodate patient/service needs.
* The ability to drive to rural locations and hold a current valid driving licence.
* Ability to manoeuvre patients using handling aids.
* Cannulation / IV Therapy administration

£39,959 to £48,117 a year per annum pro rata
