We are pleased to offer a position with the Saxmundham and North East Integrated Neighbourhood Team as an INT (Integrated Neighbourhood Team) Care Coordinator. This unique role allows you to work across health, social care, and the wider community, providing essential support and coordination to those in need.
You will be
* co-ordinating the care to keep people well at home and maximising their independence through transforming the way we work, building relationships, reducing duplication, unnecessary admissions to hospital and delivering services closer to home
* liaising with individuals and families to ensure they have access to the best possible support and services for their needs
* working in a friendly and supportive environment, alongside professionals who are committed to developing the service, such as social care teams, community health teams, statutory services and voluntary organisations, NHS colleagues, and other organisations within their designated locality.

These positions are working across organisations within the Integrated Neighbourhood Team. If you are currently on NHS Terms and Conditions, you will be able to stay on these terms. If you are a current member of Suffolk County Council, or an external applicant, you will be employed under Suffolk County Council Terms and Conditions.
Please note the post is available on a fixed term basis for East Suffolk and North Essex NHS Foundation Trust (ESNEFT) until 31 December 2025. Internal applicants currently employed by ESNEFT will be offered on a secondment basis only. Permanent employment is possible via Suffolk County Council.
