Job Summary
* Job Title: Social Worker / Caseworker

* Working Hours: Mon–Fri (44 hours/week, varies by location)
Main Responsibilities
* Developing and implementing individualised care plans for residents.
* Monitoring residents’ progress and well-being and adjusting care plans accordingly.
* Planning and conducting programmes and activities to meet residents’ care needs.
* Preparing monthly case notes and 6-monthly review reports for assigned residents.
* Ensuring adherence to SOPs and care standards in service delivery.
Requirements
* Degree or Graduate Diploma in Social Work, Psychology, or Counselling.
* 1+ year of relevant experience in social service / care settings.
* Willing to perform shift work based on duty roster.
* Able to follow structured procedures and documentation requirements.
* Require rotating shift work, which may include weekends, night shifts, and public holidays.
