Primary Function:
The RN Care Manager (RN CM) performs utilization management, case management, and discharge planning activities for acute care and extended care patients, including ambulatory and emergency department patients. The RN Care Manager collaborates with healthcare teams to assess patients' transition/discharge planning needs, actively prevent unnecessary admissions/readmissions, and facilitate timely transitions/discharges.
Job Qualifications and Specifications:
* Hired after January 01, 2016: Bachelors of Science in Nursing from an accredited institution.
* Must have and maintain a Rhode Island Registered Nurse license.
* Minimum of 3 to 5 years of related clinical experience as a Registered Nurse.
* Prior acute care Case Management experience preferred.
* Knowledge of Care Management through a Nursing Care Management Certificate or demonstrated current knowledge via pre-hire assessment.
* Certification in Case Management (CCM or ACM) must be earned within 48 months of employment if hired after January 01, 2016.
* Effective communication skills with physicians and team members, including documentation and influencing skills.
* Ability to interpret medical data.
* Knowledge of utilization management, quality management, and third-party payers is preferred.
* Training in multiple computer systems (e.g., EMR, Microsoft Office) required.
* Participation in weekend and holiday coverage rotation; flexibility to cover as needed.
