Dimensions and Context of Role
To work as a member of the Public Health Nursing team providing a service to children and their families as delegated by the Health Visitor. To participate in the implementation of The Healthy Child Programme under the direction of the Health Visitor. The post holder will work in various community settings including families' homes. This role involves being fast-tracked onto the SCPHN Health Visitor course.
Each Development Community Public Health Nurse will complete a competency framework provided by LSW and a mutually agreed learning contract. Support will be provided by a Practice Assessor and the wider PHN team. All registered Nurses, Midwives, and SCPHNs must adhere to the NMC Code of Professional Conduct, acting to justify public trust, uphold the profession's reputation, and safeguard patients' interests. They are accountable for their practice and must ensure their NMC registration is current.
Key Tasks and Responsibilities
* Work with the PHN team to achieve KPIs.
* Support families in various community settings under the guidance of the HV/SN, considering diverse social, health, and wellbeing needs.
* Plan and deliver health activities, including group work, clinics, and individual support.
* Support families using the Solihull approach in behaviour management and parenting skills, and deliver care programs for vulnerable families.
* Support breastfeeding and parent-infant relationships, aligning with UNICEF UK Baby Friendly standards.
* Understand and follow local safeguarding policies and procedures.
* Collaborate with Children Centres and Early Years settings to promote health and wellbeing.
* Support professional development of students and new staff, and participate in supervision and training activities.
* Liaise with other agencies to provide integrated support for children and families.
* Maintain accurate records and participate in research, audits, and data collection.
* Continuously review and develop professional practice, including reflective writing and gaining additional experiences.
Working relationships
Engage with individuals, families, schools, community, and multi-agency partnerships, including social care, public health, hospitals, mental health services, and educational institutions.
