What you will do:
* Conduct strengths-based assessments to understand the needs and goals of our customers.
* Collaborate with customers and their families to design and implement innovative reablement plans.
* Support the occupational therapy assistants and work alongside our supportive reablement team.
* Conduct client visits in various settings, including their homes or hospitals, to provide hands-on support.
* Maintain clear communication with all parties involved to ensure outstanding care delivery.
To succeed in this role, you must hold a qualification in Occupational Therapy, have a minimum of two years post-qualification experience, and possess a valid full UK driver’s license, as travel to client’s locations is part of the role. Registration with HCPC is vital for this position.
