The Hospice Advice & Response Team (HART) plays a vital role in delivering responsive, specialist palliative care across our community. Operating 24/7, the team provides expert advice, rapid clinical assessment, and urgent interventions to patients, families and healthcare professionals when it matters most. As Community Manager (HART), you’ll be responsible for the strategic, professional and operational leadership of our HART service, alongside oversight of the Hospice at Home (HAH) team. Working closely with the Head of Community & Dementia Services and fellow Community Manager, this role ensures seamless, high-quality care across urgent response and unplanned community services. This role requires a confident clinical leader who can balance operational delivery with compassionate, people-focused leadership. You’ll lead multidisciplinary teams, manage performance and governance, and build strong partnerships across the health and social care system to ensure patients receive safe, timely and coordinated care. Key Responsibilities § Provide visible, professional leadership to the HART and Hospice at Home teams, ensuring consistently high standards of care§ Oversee the safe and effective delivery of a 24/7 Adviceline and Rapid Response service§ Manage service performance against KPIs, quality standards and clinical governance requirements§ Lead, support and develop a multidisciplinary workforce, including line management, performance management and workforce planning§ Act as a key link between community teams, internal services and external partners to support integrated care pathways§ Champion safeguarding practice and act as Designated Safeguarding Lead§ Support service development, audits, quality improvement initiatives and change management§ Provide expert clinical leadership, advice, coaching and mentorship to staff§ Ensure compliance with CQC standards, safeguarding legislation and organisational policies About You We’re looking for an experienced and compassionate clinical leader who thrives in a fast-paced, complex community environment and is passionate about delivering outstanding palliative care. You’ll bring: Registration as a Nurse or Paramedic Significant post-registration experience, including leadership or people management Proven knowledge and experience in palliative or end-of-life care Strong clinical assessment, decision-making and triage skills Experience managing teams, performance, rotas and service delivery models Confidence working collaboratively with external partners and stakeholders A values-led approach aligned with Compassion, Accountability, Respect and Excellence The ability to lead through change, manage complexity and support staff wellbeing Leadership qualifications, independent prescribing, or advanced communication skills are desirable, but we’re equally keen to support development where needed.
