Discharging an individual onto the right care pathway requires a whole system approach. Each local health and care system has a Transfer of Care Hub that links community services across sectors (health, social care, housing, voluntary sector) both physically and virtually. The team ensures patients receive the right onward care and support regardless of where they reside.
