Main duties of the job
A Pharmacy Care Co-ordinator supports patients with their medications within a Primary Care Setting, Alma Medical Centre. They work with pharmacists, GPs, and other healthcare professionals to ensure patients receive the right medications, understand how to take them, and are supported in managing their medication regimens effectively. This includes coordinating medication reviews, managing repeat prescriptions, and facilitating communication between different healthcare providers.

Job responsibilities
Identifying and addressing issues related to medication use, such as polypharmacy (multiple medications) or potential side effects, to improve patient outcomes.
Providing information and education to patients about their medications, helping them understand how to take them correctly and manage potential side effects.
Communication and Coordination:
Facilitating communication between patients, doctors, pharmacists, and other healthcare professionals to ensure smooth transitions of care and timely follow-up on medication-related issues.
Administrative Tasks:
Handling repeat prescription requests, managing medication reviews, and assisting with medication audits and safety monitoring.
Referral and Signposting:
Directing patients to appropriate services, such as community pharmacies or specialist clinics, when needed.
Medication Safety:
Monitoring patients on high-risk medications, such as those requiring regular blood tests (DMARDs, anticoagulants), and ensuring appropriate follow-up.
Working Relationships:
Works closely with clinical pharmacists, pharmacy technicians, GPs, nurses, and other healthcare professionals.
Community Pharmacies:
Collaborates with local pharmacies to ensure smooth medication supply and management.
Patients and Carers:
Provides direct support and education to patients and their families regarding medication management.
