Salary: BAND 8A - Up to £60,000
About the Role
We’re looking for an enthusiastic and proactive Clinical Pharmacist to join a friendly, forward-thinking GP practice on the beautiful South Coast. You’ll work closely with GPs, nurses, and other healthcare professionals to optimise patient medicines, provide structured medication reviews, and support safe, evidence-based prescribing.
Key Responsibilities
* Conduct structured medication reviews, particularly for patients with long-term conditions or polypharmacy.
* Support safe and effective prescribing, including monitoring high-risk medicines and managing repeat prescriptions.
* Provide patient consultations (face-to-face, telephone, or video).
* Promote deprescribing and reduce medication-related harm where appropriate.
* Prescribe within your competence.
* Manage minor conditions or chronic disease reviews under agreed protocols.
* Lead audits, QOF, IIF, and local prescribing incentive schemes.
* Collaborate with community pharmacies, care homes, and NHS teams.
* Advise on cost-effective prescribing in line with local guidance.
Education & Development
* Provide training or updates for practice staff.
* Mentor pharmacy technicians, if appropriate.
* Keep up to date with clinical guidance and professional development opportunities.
* GPhC-registered pharmacist
* Strong clinical knowledge and patient-focused communication
* Experience in primary care, hospital, or community pharmacy
* Commitment to CPD and evidence-based practice
* Completion of the CPPE Primary Care Pharmacy Education Pathway
* Experience with SystmOne and e-prescribing tools
* Familiarity with QOF, IIF, or other NHS frameworks
