Trust Grade Doctor (SpR level) in Respiratory Medicine

We are looking for an enthusiastic capable doctor to join our fantastic respiratory team at Charing Cross Hospital, Imperial College Healthcare NHS Trust, at a specialist registrar level. The job is for a 0.5 FTE (equivalent of 2.5 days per week + 0.5 FTE on calls). It will suit a doctor with full Royal College of Physicians membership (i.e. MRCP 1, 2, PACES), enthusiastic about respiratory medicine and committed to the general medical take to join a supportive department. The post has arisen from a national training number gap and the post holder will join a warm and welcoming team. Respiratory experience is desirable but not essential and will be gained during the post. Working days are likely to be Thursdays and Fridays with alternate Mondays with an additional on-call commitment. However, there is scope for some flexibility in determining working pattern.

Main duties of the job

As part of the respiratory team, the post holder (along with the other registrars, SHOs and FY1s) will be responsible for the ward-based care of 19 patients on 4 South ward, including a 4-bedded ARU. They will also periodically work on the Acute Medicine ward, looking after recently admitted patients. The post holder will be on the on call rota (0.5 FTE), clerking new admissions and providing out of hours (evening and weekend) ward cover. There is no obligation to attend respiratory outpatient clinics or procedure lists (bronchoscopy, pleural) but where there is interest, this is encouraged and there are plenty of opportunities to do so.

Job responsibilities

The respiratory post holder will work as part of a consultant-led team looking after inpatients on the respiratory ward (4 South). The 19 patients on the respiratory ward (including four Acute Respiratory Unit beds) are looked after by a medical team comprising of 7 consultants, 4 registrars (including this post), 3 SHO grade doctors and 2 foundation year 1 doctors. The ward is the designated area for caring for patients with chest drains, tracheostomies, high flow nasal oxygen and non‑invasive ventilation. The patients have conditions such as lung cancer, COPD, asthma, respiratory infections (e.g. pneumonia, TB, bronchiectasis), pleural disease (pneumothorax, effusions and empyemas), respiratory failure, interstitial lung disease and other general medical conditions.

The post holder will participate in daily board and ward rounds, complete jobs from the ward round, and attend weekly radiology meetings. They will be expected to be able to:

* Take a history and examine a patient
* Identify and synthesise problems
* Prescribe safely
* Keep an accurate and relevant medical record
* Manage time and clinical priorities effectively
* Communicate effectively with patients, relatives and colleagues
* Use evidence, guidelines and audit to benefit patient care
* Act in a professional manner always
* Cope with ethical and legal issues which occur during the management of patients with general medical problems
* Teach more junior colleagues and medical students


Person Specification

* Must have at least completed foundation and internal medicine training or equivalent
* At least 6 months experience working clinically in the NHS as a doctor (unpaid posts such as observerships do not count).
* Experience of working for a minimum of 4 months in a medical specialty in the UK
* Experience of working in respiratory medicine
* Experience in using NIV or CPAP
* Recent experience (in the last 2 years) working within a medical specialty in the NHS (in particular respiratory medicine).


Quality Improvement Project (QIP) / Audit

* Evidence of participating in and/or leading a QIP/audit
* Completion of QIP/ audit cycle with evidence of improvements being implemented


Research

* Evidence of participating in research including publication(s), particularly in a relevant field (e.g. respiratory)


Qualifications & role suitability

* On GMC Register with licence to practice
* Advanced Life Support
* Full MRCP (part 1, 2 and PACES)


Clinical Knowledge

* Competence in basic clinical procedures
* Ability to manage chronic and acute respiratory conditions Aware of own limitations, consults senior colleagues appropriately
* Experience of being a medical SpR and managing the acute medical take
* Thoracic ultrasound emergency or primary operator competencies
* Experience conducting outpatient, and particularly respiratory, clinics
