Job summary Interview Date: 10th June 2026 As a Patient Flow coordinator, you will be based in the RightCare department and will deal with referrals taken electronically and over the telephone. You will be part of a wider team including Nurses, administrative officers, Social Care and Intermediate Care staff. You will send referrals to a range of health and social care providers, including Care Homes; you will follow up the referral to confirm service provision and let the relevant teams know. The aim of the service is to ensure that the right care is provided, at the right time and in the right place. You will work closely with health, social care teams and professionals from primary, secondary and community care supporting the RightCare nursing and coordination staff access care or other more suitable alternatives to hospital admission that will meet the needs of the service user. Main duties of the job RightCare is Barnsleys Single point of Access for Health and Social Care staff supporting patient flow across Hospital and Community, coordinating admissions and discharges from Intermediate Care beds and Services, arranging support on discharge from hospital for Barnsley Residents. You should have excellent interpersonal, communication, decision making and influencing skills in order to ensure the highest standards of care are achieved.
