Job Summary
The Role's Main Focus Will Be To Provide High Quality Planned And Proactive Care For Care Home Patients In a Weekly Ward Round; Including But Not Limited To:
* supporting the practice with ongoing management of care home patients by reviewing patients and amending Personalised Care & Support Plans and RESPECT forms.
* supporting care home patients with anticipatory care discussions and planning.
* providing acute medical treatment for care home patients with undifferentiated and undiagnosed problems
* assessing and managing patients with undifferentiated and undiagnosed health care needs including investigating, treating, and referring patients as appropriate.
* Independently prescribing (where applicable to the role) and review appropriate medications in accordance with local policies and evidence based medical practice and work collaboratively with the wider team to support patients and to promote medication adherence.
* ordering investigations, follow-up results and arranging appropriate management including seeking advice where appropriate.
* Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person.
* Supporting practices with ward rounds and MDTs.
Job Responsibilities
We are looking for an ANP or ACP to work 3 days per week carrying out ward rounds to our care homes. You will work together with the GP who has responsibility for the patients residing in the care homes, and you will have the expert help of a dedicated care home coordinator who will manage the administrate and planning side of your rounds. The ideal candidate will be an excellent communicator, organised and efficient and able to work independently.

Qualifications
Essential
* The successful candidate will have or working towards a Post Graduate Degree Qualification in their field, Clinical Practice or equivalent qualification, with evidence of academic study and practical experience of situations typically encountered in a primary care setting.
* You will need to be registered with a professional body.
* It is a requirement for the candidate to have attained a post graduate qualification in Prescribing or be working towards qualification.
* To remain up to date with all mandatory training.
* To maintain up-to-date knowledge of safeguarding adult guidance to ensure that those at risks of abuse/suffering abuse are identified and the necessary process to protect them is followed.
