The post holder will be part of a dynamic team of clinicians responsible for developing and improving services for people over 18 years with common mental health problems in primary care. The role involves working across designated GP practices within primary care networks and attending weekly integrated care team meetings.
The post holder will provide consultation and liaison services across primary and secondary care sectors, facilitating access to appropriate services. Responsibilities include mental health screening assessments, risk assessments, medication monitoring, telephone triage, signposting, referrals, and brief interventions for individuals with common mental health issues.
Additionally, the role involves providing support, education, and problem-solving advice to primary care team members, working across various geographical settings, and undertaking home visits.

* Manage administrative, preceptor, and clinical supervisor roles as required.
* Maintain accurate clinical notes and activity records, participate in audits and research.
* Contribute to developing services to meet mental health needs in primary care, ensuring they are equitable, non-stigmatising, and responsive.
* Maintain and enhance knowledge and skills in mental health, acting as a role model and resource for junior staff and students.

Person specification
* Registered on a Nursing Midwifery Council Register (Sub Part 1 RN3, Registered Nurse Mental Health).
* Evidence of ongoing professional development.
* Willingness to undertake non-medical prescribing.
* Post-registration experience in community mental health services.
* Experience working collaboratively within multidisciplinary/multi-agency teams.
* Ability to manage and organize own workload.
* Skills in assessing and formulating intervention plans for adults with mental health problems.
* Knowledge of evidence-based practices and NICE guidelines.
* Understanding of clinical governance and safeguarding principles.
* Excellent oral and written communication skills.
* Training in teaching, clinical supervision, motivational interviewing, and solution-focused working.
* Experience supporting staff in clinical service provision.
* Management or leadership experience.
* Experience delivering innovative clinical practices.
