We are looking to recruit an enthusiastic Clinical Lead to join the Rural Alliance Care Community Team in rural western Cheshire.
The multidisciplinary team includes Team Manager, Enhanced Clinical Practitioner, Clinical Leads, Community Nurses, AHPs, and other specialist professionals.
The role requires an enthusiastic, self‑motivated leader who provides confident support and clinical leadership to clinicians and collaborates with other professionals.
Excellent communication skills are essential, with the ability to work with patients and their families/carers and to safeguard individuals at risk from harm and abuse.
The service is undertaking a review of working hours and may require work beyond 17:00.
Key responsibilities:
* Provide a highly visible clinical profile with leadership and clinical oversight of a designated community caseload.
* Work collaboratively with the wider health, social care and third‑sector partner agencies to meet the needs of patients.
* Ensure patients receive the right care by the right person at the right time and in the right place.
* Place the patient at the centre of all decisions, reinforcing a streamlined patient journey with minimal avoidable delays, and emphasis on independence and quality of care.
* Provide direction within the caseload in consideration of the impact of current health and social care agendas and local initiatives.
* Be aware of local public health community profiles and how this influences the required care for the population.
* Understand the skills required from the Community Care Team according to community profiles.
* Deliver person‑centred care focusing on defined populations through case‑load management and monitoring of care standards.
* Provide clinical leadership.
