Job overview
These are posts based in North Wales, and involve combining sessions in transfer and retrieval medicine, critical care and/or anaesthesia. This is a full-time post with out of hour’s commitments. The job plan is negotiable, depending on individual’s interests and service requirements, and split into 2 components. A typical job plan will include:
1. Approximately 50% of shifts will be with ACCTS-North
2. Approximately 50% in Critical Care or Anaesthesia at Ysbyty Glan Clwyd
The job is suitable for doctors who have completed core training in the Adult Critical Care Transfer Service Cymru (ACCTS), intensive care or anaesthesia and have a minimum of 18 months of experience in anaesthesia.
This is a unique opportunity to be part of the development of a national Critical Care Transfer Service for the people of Wales. Established to provide high-quality intensive care during the inter-hospital transfer of the critically ill patient.
ACCTS Cymru, in partnership with the Welsh Critical Care Network and EMRTS Cymru, is seeking experienced, motivated, and enthusiastic senior retrieval and transfer medicine specialists to support the development of this bespoke service for Wales.
There are around 600 critical care (Level 2 and Level 3) transfers in Wales per annum. It is anticipated that the number of transfers will increase over the coming years as various regional networks become established and mature, as well as an increasing demand for critical care services more generally.
Main duties of the job
ACCTS is a predominately road-based service deployed with dedicated critical care transfer vehicles and equipment. The Retrieval and Transfer physician will deploy in a team alongside a support worker and a retrieval and transfer practitioner drawn from a pool of nurses and allied health professionals with extensive intensive care experience and specific training in inter-facility critical care transfer. ACCTS in partnership with WAA/EMRTS will operate a rotary wing model of retrieval and transfer to support long-distance transfers.
Upon completion of appropriate training and a period of targeted clinical experience, it would be expected that you would then take up a full part in the duty rota. The ability to work both day and night shifts is an essential part of the role.
Whilst a base location will be agreed upon with the successful candidate there may be a need to travel to our various bases, across Wales for education and service delivery requirements and all candidates would be expected to contribute to twilight or overnight cover.
The departments of Anaesthesia and Intensive Care at Glan Clwyd Hospital will offer a training program with a view to some candidates staying on for a further 2 years to complete the core training curriculum in anesthesia, to allow application for entry into specialist training in Anaesthesia or Intensive Care Medicine.

To qualify for this exciting role, you must be a doctor with ST4+ equivalent training in Intensive Care Medicine / Anaesthesia who is fully registered with the GMC and who has significant demonstrable experience in the transfer care setting, aligned to the personal specification. It is essential that you are ambitious, highly motivated, and able to remain calm under pressure.
Selection for this rewarding role is demanding and consists of several clinical scenario assessments as well as group work and a formal interview. An assessment event will be held at one of the ACCTS operational bases at a date to be confirmed.

Essential criteria
3. Full GMC Registration with license to practice and up to date appraisal
4. Successful completion of relevant life support / resuscitation / trauma courses previously
5. A British driving license
Desirable criteria
6. Completed intermediate level training / Stage 2 training in Anaesthesia/ICM OR ST5 equivalent experience
7. Substantive Consultant post within Wales
8. Diploma in Retrieval and Transfer Medicine
9. Clinical fellowship in a Retrieval System (Overseas/Military/UK)
10. Higher Degree (MD/MSc)
11. Post graduate qualification in medical education
12. Experience in FAST scanning or bedside echocardiography
Knowledge and Skills
Essential criteria
13. Competence in management of acute illness and injury in adults
14. Competent in the assessment, resuscitation and stabilisation of critically unwell patients including emergency anaesthesia, ventilation and invasive monitoring
15. Sound judgement and decision making
16. Ability to manage complex work and prioritise effectively
17. Good understanding of the principles of clinical governance
Desirable criteria
18. Previous attendance on an EMRTS/ACCTS clinical attendant scheme, and/or prior engagement with the EMRTS service. (Such as attendance at EMRTS governance days, training events etc.)
19. Evidence of relevant teaching/instructing in critical care or transfer
Experience within 5 years
Essential criteria
20. Demonstrable interest in and understanding of retrieval and transfer medicine
21. Extensive experience of inter- and intra-hospital critical care transfers
22. Experience in post-graduate / multidisciplinary education
23. Experience of critical care transfer and retrieval and/or pre-hospital emergency medicine in dedicated service
Desirable criteria
24. Prior experience with evidence of working within an established pre-hospital aeromedical or road-based service
25. Prior experience with evidence of working within an established secondary retrieval service
26. Prior experience working in within a trauma centre (experience of major trauma, major haemorrhage, massive transfusion)
27. Experience using simulation for education
28. Experience as a clinical tutor or lecturer at post graduate level
