This role is offered on a fixed term/secondment basis for 12 months. The hours are part-time, 30 hours per week.
Due to the current post holder commencing maternity leave, we have an exciting opportunity for an enthusiastic and skilled Physiotherapist to join the Integrated Community Team (ICT) in the Stroud District on a fixed term contract until August 2027.
The ICT consists of Community Nursing, Physiotherapy, Occupational Therapy and the Home First/Reablement Team. We work collaboratively with the wider MDT within our service and our Integrated Care System (ICS) partners to enable individuals to remain as healthy and independent as possible within their own homes.

Main duties of the job
* Manage a specialised clinical caseload and independently lead clinical practice within the community.
* Triage and manage work within the Stroud Referral Centre on a rota basis.
* Formulate, evaluate and monitor specialised rehabilitation care packages, including Home First services.
* Lead urgent responses to priority cases and coordinate multidisciplinary team members.
* Supervise, train and provide performance support to physiotherapists, physiotherapy technicians/assistants, reablement workers, students and community care workers.
* Coordinate, allocate and organise the work of junior and assistant staff to meet service priorities.
Qualifications
* Diploma/degree in Physiotherapy
* HCPC registration
* Full driving licence and use of a vehicle
* Membership to a relevant professional body e.g. CSP
Length and/or nature of experience
* Broad range of clinical experience across a variety of clinical areas
* Experience of managing junior members of staff and rehabilitation assistants
* Evidence of working unsupervised
* Evidence of working across multi‑disciplinary and multi‑agency boundaries
Professional/Managerial/Specialist knowledge
* Ability to deliver community rehabilitation and treatment of patients with complex clinical presentations
* Demonstrated knowledge of the Home First pathway
* Experience of performance management
