Self Management Facilitator/ Call Handler

Job Overview

This is a fixed-term/secondment position lasting 8 months until 31/03/2026, to meet the needs of the service. If you are interested in applying for the secondment, you must obtain permission from your current line manager prior to applying.

Are you eager to be part of an exciting team within the health board? Do you want to improve the care and experience of all age patients accessing Planned Care services? Are you looking for an opportunity to play a pivotal role as the first point of contact for patient services at Betsi Cadwaladr University Health Board?

As a Single Point of Contact (SPOC) and Self Care Service Facilitator, you will provide high-quality service to the SPOC team, supporting the delivery of the 3P's. We offer a supportive working environment with a strong focus on professional development, mentorship, and opportunities for personal growth.

Main duties of the job

This role involves supporting patients as a Health Coach and facilitating Self Management education, particularly for the Health and Wellbeing courses. The position is predominantly home-based, utilizing a Teams-enabled laptop.

Proficiency in Welsh is desirable; applicants fluent in English and/or Welsh are equally encouraged to apply.

* Desirable: Experience in a professional caring role for individuals experiencing life changes due to long-term conditions.

Skills

* Essential: Computer literacy, especially with Microsoft Teams.
* Desirable: ECDL qualification.
