Self Management Facilitator/ Call Handler
Job Overview
This is a fixed-term/secondment position lasting 8 months until 31/03/2026, to meet the needs of the service. If you are interested in applying for the secondment, you must obtain permission from your current line manager prior to applying.

As a Single Point of Contact (SPOC) and Self Care Service Facilitator, you will provide high-quality service to the SPOC team, supporting the delivery of the 3P's. Main duties of the job
This role involves supporting patients as a Health Coach and facilitating Self Management education, particularly for the Health and Wellbeing courses. The position is predominantly home-based, utilizing a Teams-enabled laptop.
Proficiency in Welsh is desirable; applicants fluent in English and/or Welsh are equally encouraged to apply.

Skills
* Essential: Computer literacy, especially with Microsoft Teams.
* Desirable: ECDL qualification.
