''An exciting and innovative opportunity has arisen within Keswick and Solway Integrated Care Community (ICC). We are a seeking a Care Coordinator to join ' The Your Health Matters Team'.
The post holder will be based within the ICC and supported by the therapists in the well established 'Your Health Matters Team'. You will work closely with GP practices within the Primary Care Network (PCN) as well as ICC colleagues in this role.
They will identify and work with a cohort of people to support their personalised care requirements using a range of interventions. Training for the role will be provided,
Further training opportunities area available as Assistant Practitioner and/or Nurse Associate for interested candidates once established in the role.
Main duties of the job
The Care Coordinator will primarily work with the following patients ;
* Moderate to severe frailty
* Individuals who are at high risk of admission to hospital and are relying on unplanned care
* Long Term Health Conditions
They will provide proactive care aimed to maximise their health and well-being and prevent unnecessary or unscheduled urgent care. The anticipatory care team will be lead by an occupational therapist and will work closely with all other health professionals including district nurses, physiotherapists, rehab assistants, assistant practitioners, pharmacists, speech and language, social care and 3rd sector services.

Person Specification
Qualifications
* NVQ2 in Rehabilitation, Promoting Independence or Care or equivalent.
* Prepared to undertake NVQ3 in Diagnostic and Therapeutic Care within 3 years of appointment
* Evidence of ongoing training and personal development
* Care Certificate or the willingness and ability to undertake (applicable for all non-registered support workers that provide health care)
* Personalised Care Institute certification or the willingness and ability to undertake (applicable for all care coordinator roles)
* NVQ3 in Health and Social Care or a Diploma in health and social care activity
* Enrolled in, undertaking or qualified from appropriate training as set out by the Personalised Care Institute
* Health trainer
* Mental capacity training
* Mental health first aider
Knowledge
* Knowledge of personal care, function and mobility, nutrition, privacy and dignity
* Knowledge and understanding of information governance and confidentiality
* Knowledge and understanding of how community services support and maintain personal independence
* An understanding of how to support, with empathy and encouragement, individuals who may be unwell or have reduced functional capabilities.
* Understanding of equality and diversity issues and how this affects patients, visitors and staff
* Awareness of clinical governance
