The Role

Are you passionate about delivering compassionate, person-centred care to older adults? University Hospital Wishaw is seeking dedicated and enthusiastic healthcare professionals to join our new dynamic Frailty Unit.

Our Frailty Unit provides specialist multidisciplinary care for older people living with frailty, focusing on early assessment, rehabilitation, and safe discharge planning whilst applying home first principles. You will work within a collaborative team, benefit from ongoing professional development, and contribute to service improvement initiatives that enhance patient outcomes.

As a valued member of our multidisciplinary team, you will work under the supervision of registered therapy staff, using evidence based practice and client centred principles, to carry out assess, treat and discharge within defined parameters and scope of practice. You will be expected to develop relevant clinical skill, knowledge and experience with access to supervision and must demonstrate competence within all four pillars of practice.

You will liaise with family, carers and other agencies to ensure assessment and care is provided in the best possible setting and alternatives to hospital based care such as community based care is utilised as well as working alongside existing MDT teams, to provide advice and recommended solutions regarding the different options to support frailty positive patients and their timely discharge from hospital.

Full and part time hours will be considered.

What we'll need you to bring

1. Registered Nurse with valid UK NMC Registration
2. Be a car driver with a full, valid UK/EU/EEA driving licence.
3. You will be required to demonstrate excellent team working skills with the ability to work using your own initiative
4. Effective listening, communication and interpersonal skills
5. Ability to work with people and as part of a multidisciplinary team
6. Excellent time management skills
7. Working knowledge of basic information technology
