Overview
Advanced Clinical Practitioner – Rapid Response Team (RRT) with Waltham Forest Adults Community Health Services Unplanned Care. The Rapid Response Team provides urgent multidisciplinary support to prevent avoidable hospital admissions, 24/7. The team works across the borough to assess and manage patients in the community with escalating health and social care needs.
Responsibilities
* Work collaboratively with health and social care professionals across the borough and Integrated Care System.
* Lead clinical decision making and undertake enhanced holistic assessment of patient/family needs in partnership with the patient and their family/carer.
* Triage referrals from other agencies or disciplines.
* Coordinate intensive and complex nursing care with other multi-disciplinary agencies.
* Provide line management supervision to members of the Rapid Response multidisciplinary team.
* Participate in and complete managerial and clinical supervision.
* Contribute to audits within the service and support Datix investigations / Root Cause Analysis / Serious Incidents and lessons learnt.
* Support and mentor pre-registration learners from a variety of disciplines.
* Carry out joint visits with MDT colleagues to promote holistic person-centred assessment and approach to care.
* Be a driver and have access to a car.
Qualifications and requirements
* Acute Care Experience Required.
* Proficient in advanced clinical skills and knowledge with the ability to assess physical and psychosocial needs and develop a comprehensive and concordant plan of care with the patient, carers and the wider MDT as needed.
* Applicant to have management experience.
* Be able to work collaboratively across services and carry out joint visits with MDT colleagues.
Other information
* Probationary period applies; internal applicants may be exempt.
* Starting with NELFT; comprehensive induction on first Monday of each month, including mandatory training and equipment allocation (induction held at Trust head office in Rainham, Essex).
* High Cost Area Supplement of 15% of basic salary (minimum £4,313; maximum £5,436 per annum pro rata).
* COVID-19 vaccination: staff are encouraged to be vaccinated and boosted.
