Job Summary
We are seeking an enthusiastic Senior Community Practitioner (Nurses) to join the Nottinghamshire Virtual Ward! One full‑time position is available within a collaborative system‑wide Virtual Ward service across Nottinghamshire Healthcare, NUH, Sherwood Forest Hospitals, Doncaster & Bassetlaw Teaching Hospitals and CityCare.
As a Band 6 Senior Community Practitioner you will provide timely assessments, person‑centred care, and advanced clinical decision‑making in patients homes, working autonomously and within an established MDT.
Responsibilities
* Deliver timely, person‑centred assessments and provide high‑quality clinical care to patients on the Virtual Ward for up to 14 days.
* Work autonomously and within the MDT (including Matrons, Nurses, AHPs and support staff) to prevent unnecessary hospital admissions and support safe early discharge.
* Interpret remote monitoring data, undertake face‑to‑face clinical reviews, identify deterioration and initiate appropriate interventions or escalation.
* Lead and coordinate safe, effective discharge planning and contribute to daily MDT discussions on treatment and risk.
* Support junior staff through supervision, role‑modelling and clinical guidance.
* Maintain accurate documentation on SystmOne and uphold professional standards, governance, and patient safety.
* Work flexibly across respiratory, frailty and admission avoidance pathways, following lone‑worker and dynamic risk assessment procedures.
Person Specification
Knowledge (Essential)
* Awareness of the NHS Modernisation Agenda
* Case management for complex patients
* Ability to seek out current best practice based on research and evidence‑based enquiry
* Knowledge of the nursing 6 cs
* Understanding of clinical risk assessments and risk management
* Ability to deliver joint managed care
Experience (Essential)
* Post registration experience
* Experience of working autonomously
* Acute care experience
* Community or primary care experience
* Team Leadership
* Caseload Management and experience of triaging patients with a range of clinical conditions
* Multi‑disciplinary working
* Experience of supervising and appraising staff
Qualifications (Essential)
* Registered general nurse (Level 1)
* Able to demonstrate management competencies
* Evidence of working at a senior level
* Degree level qualification or equivalent knowledge and experience in a health‑related subject (e.g., Specialist Practice Degree or Leadership Qualification)
* Advanced clinical assessment evidence (practical evidence or courses)
