Overview
This role will support families with Learning Disabilities (LD) and Special Educational Needs and Disabilities (SEND) and work closely with schools (including SENCOs), family hubs, and children's services to ensure inclusive, coordinated support. Social Prescribing Link Workers (SPLWs) provide a vital non-clinical option for health and care professionals to support patients whose needs cannot be met through traditional medical interventions alone. Our SPLWs empower individuals and families to take control of their health and wellbeing by focusing on 'what matters to me' and connecting them to community-based resources. By building trusted relationships and facilitating access to local services, SPLWs help create sustainable change and reduce health inequalities.

Responsibilities
The SPLW for SEND Families will receive and action referrals from GPs, SENCOs, schools, family hubs, children's services, and wider adult social prescribing teams. We are seeking a caring, empathetic, and proactive individual to support people of all ages, particularly SEND families in educational settings, to improve their health and wellbeing. You will work collaboratively with GP practices, schools, family hubs, and voluntary sector organisations to co-produce personalised support plans and connect individuals to community resources. This includes building strong networks with education and family support services, facilitating group sessions, and ensuring families feel supported throughout their journey. You will manage a varied caseload, work autonomously, and seek advice when needed, while promoting inclusion and safeguarding at all times.
About the Service
The North West Leicestershire GP Federation consists of 12 GP practices serving approximately 116,000 patients and operates the North West Leicestershire Primary Care Network (PCN). Our Social Prescribing team works across three neighbourhoods to provide holistic, community-based support that complements clinical care. As part of our commitment to reducing health inequalities, we are expanding our service to focus on SEND families and school-based collaboration, ensuring families receive timely, integrated support.
