To comply with safeguarding regulations, we are only able to accept applications from candidates who are over 18.

The post holder will work as part of a multidisciplinary team to deliver patient-centred medicines optimisation and technical services within Humber Teaching NHS Foundation Trust.

Activities will include reconciling medicines, assessment of Patients own drugs, maintaining appropriate levels of stock control, ensuring cost effective use of medicines, timely and accurately labelling and dispensing medicines, providing advice in relation to safe and secure handling of medicines to patients and colleagues, counselling and educating patients/carers on their medicines, completing medicines related audits and deliver education and support to maximise patient's independence with medicines.

Work towards the completion of the selected apprenticeship standard whilst working closely with the team to gain the required skills, knowledge and behaviours for the role.

The post holder will be required to participate in any medicines optimisation and safety initiatives within the directorate.

The post holder will need to be trained in DMI approved techniques (completion of 5-day course).


Main duties of the job

The main duties of the role will include -

* To assist with safe, appropriate, efficient and cost-effective use of medicines
* To facilitate an integrated provision of pharmaceutical care including prescribing, ordering, transport, preparation, administration, storage and disposal of medicines
* To monitor effective medicine stock control and storage on the unit by carrying out regular audits and providing advice to the unit and team on safe, secure and appropriate storage of medicines.
* To improve pharmaceutical care by piloting and introducing new systems for the use of medicines in appropriate settings.
* To ensure effective management of the pharmacy ordering system and stock control.
* To label and dispense non-stock medicines, medicines used for Self-administration and medicines for leave or discharge in line with the Trust process and procedures.
* Providing routine information and advice to patient/carers and colleagues in regard to safe and secure handling of medication
* To assist with discharge planning and ensure post discharge plans are in place.
* Accurately completing medicines reconciliation for patients following transfer of care.
* To ensure pharmaceutical wasteis handled safely and appropriately. Assisting with or arranging destruction, storage and collection)
* To plan and organise your workload
* Any other duties as reasonably required by the line manager. physical health equipment monitoring, phlebotomy, medicines administration and the Health Improvement Profile (HIP)

Person Specification


Qualifications and Knowledge

* 5 GCSEs (4-9 or equivalent) including Math, English and 2 Science


Experience

* Experience working in a pharmacy area
* Experience working as a pharmacy technician within a hospital or in a primary care environment


Skills and Competencies

* Good communicator, both verbally and in writing
* Ability to work effectively in a team
* Good analytical skills
* Reliable team worker
* Self-confident
