Background Information:
Active Living is a unique, award-winning Exercise Referral scheme designed to support people to increase their physical activity levels and improve their health. This initiative is a partnership between Abbeycroft, West Suffolk NHS Foundation Trust and Allied Health Professionals. Experience instructors will support patients through a programme of activity lasting either 12 weeks or 24 weeks, with follow-on options available for everyone to promote long term behaviour change. Our person-centred approach will ensure all aspects of health relevant to the client are considered and supported.
Main Purpose of Job:
A dynamic and motivational Active Living Practitioner to provide health and wellbeing support to clients accessing the service, supporting lasting lifestyle changes to our clients.
You will support by responding to general queries, problem solving, and ensuring quality of delivery, whilst escalating matters to the Assistant Health and Wellbeing Manager where appropriate. You will complete health assessments, consultations and inductions including the provision of safe and effective exercise programmes to support clients with more complex needs which may include various long term health conditions and/or issues with accessibility.
As a Practitioner you will be required to coordinate Active Living scheme services and the delivery of required sessions/classes. You will support and empower the client to use their skills and networks to overcome barriers and make positive changes to improve their health and wellbeing. You will always ensure excellent service is adhered to.

Responsibilities:
* Provide triage, health assessments and inductions of clients accessing the Active Living service in your locality, including the provision of safe and effective exercise programmes as required.
* Attend local events and locality meetings where appropriate to form working partnerships with community stakeholders. Work closely and forge strong links with partners across the locality at place and neighbourhood level to refer and signpost the client to ensure suitable support is provided, including but not limited to local authorities, the integrated care board and primary/secondary care, leisure providers, and Voluntary, Community, Faith, and Social Enterprise organisations (VCFSE).
* Identify and support clients with complex needs to access the most appropriate service and sustainable physical activity to support lasting lifestyle changes.
* Complete re-assessments and evaluations of clients to monitor their progress, ensuring that appropriate data is collected and recorded and reporting on outcomes as required.
* Support the Assistant Health and Wellbeing Manager to manage data entry into the case management system ensuring accuracy and efficiency, whilst contributing to the measurement of change and improvement in health behaviours using relevant outcome measures.
* Be a role model for the delivery team to ensure consistency and quality of delivery and to promote best practice, whilst championing and contributing to a positive, open culture where questions and learning are shared, enabling continuous development both of individuals and the service.
* Work closely with each centre Practitioner to promote consistency of delivery and best practice.
* Understand the wider determinants of health and barriers to healthy lifestyles and reduce health inequalities impacting on health behaviours.
* Undertake additional training and development opportunities to ensure continual professional development and to boost the schemes provision.
* To undertake such other duties as may reasonably be required compatible with and/or arising from those listed above.
