You will effectively work within the reablement team to deliver a high quality reablement service with the aim of promoting and sustaining people’s independence living in the community. Supervise staff / mentor personal development of Reablement workers A driving licence is essential criteria for this community-based role. Complete service users journeys well which coordinated and complies with organisational policies, procedures and Care Quality Commission regulated guidance. To ensure that the reablement service achieves the best quality outcomes for service users, their carers and families.
