What you will do:
Carry out assessments and support planning under Birmingham City Council’s Conversations model.
Navigate complex caseloads involving housing challenges, safeguarding, substance misuse, and risk management.
Facilitate safe and stable transitions from hospital discharge into appropriate housing pathways.
Coordinate with healthcare professionals, housing officers, and other stakeholders to deliver holistic support plans.
Maintain accurate and timely documentation aligned with statutory and organisational standards.
