Responsibilities
* Deliver effective, evidence-based care to individuals experiencing substance misuse issues.
* Conduct comprehensive assessments, develop tailored care plans, and provide ongoing reviews.
* Facilitate therapeutic group sessions and one-to-one interventions promoting recovery and wellbeing.
* Work collaboratively with multidisciplinary teams to provide holistic, coordinated care.
* Maintain accurate and confidential records in line with data protection and clinical governance standards.
* Engage in community outreach to raise awareness and provide education on substance misuse and recovery services.
* Design and implement innovative programmes and interventions to support clients on their recovery journey.
