Main duties of the job

Staff working within Primary Care will:

1. Provide an accessible individual patient assessment, treatment and referral service
2. Monitor the ongoing health status of patients and address identified issues
3. Work with patients to influence the impact on their health arising from current lifestyle patterns/choices
4. Implement effective discharge plans for transfer or release locations
5. Co-ordinate multi-disciplinary clinical practice to provide focused integrated care and treatment programmes/packages.
6. Work collaboratively with staff of other disciplines e.g. officers and governors to provide integrated and coordinated treatment plans.

21. Triage patient needs in accordance with specific standard operating procedures and national policy e.g. NICE guidance
22. Assess, plan and implement care for patients within each care/treatment pathway in order to promote wellbeing
23. Facilitate the use of evidence based practice to ensure that clinical care is both effective and appropriate
24. Carry out general nursing duties
25. Maintain contemporaneous records
26. Support and make a professional contribution to the development of integrated healthcare
27. Act as a preceptor/mentor in accordance with experience

HMP Send is situated within an acute rural area, candidates will require their own transport to access, there is no direct public transport links to and from the establishment.

Our healthcare services work to a 7-day week working model, to ensure our patient's healthcare is managed and maintained to the highest of standards at all times.

All of our HMPPS sites offer free use of the onsite leisure facilities and free parking for staff.
