Health Visitor

We have a vacancy for a Health Visitor within Cambridgeshire to work as part of the 0-19 Healthy Child Programme.

Main duties of the job

To work as part of a team leading the 0-19 Healthy Child Programme. The Health Visitor promotes good health and prevents illness in children, families, and communities.

Health Visitors raise awareness of unmet health needs, and initiate, develop, implement and evaluate care plans.

They work in partnership with families, primary care, statutory and voluntary services, and other partner agencies to facilitate health‑enhancing activities.

Health Visitors use and establish networks within the community to inform, influence and implement policies that affect health outcomes.

The Health Visitor acts as a named Health Visitor within a zone of a corporate caseload and as a link Health Visitor for identified GP practices, delivering the Healthy Child Programme in Cambridgeshire.


Job responsibilities

Main Duties and Responsibilities.

Identify the health needs of children and young people, their families, and the community, and contribute to developing a strategy to address identified needs.

Use knowledge and skills to assess, plan, implement and evaluate packages of care.

Identify health needs within a geographical corporate caseload/community.

Clinically triage the SPA as required in accordance with agreed procedures.

Promote local and national public health priorities.

Refer to other professionals and agencies when responding to health needs assessments.

Support vulnerable or at‑risk children, young people and families, adhere to safeguarding procedures, and participate in child protection case conferences.

Participate in statutory safeguarding processes and provide witness statements, court reports, and attend court as required.

Undertake care programmes in partnership with parents, foster parents and other agencies to safeguard children and minimise risk.

Identify children with special educational needs and disabilities, facilitate support and referrals, and act as a key worker when indicated.

Provide cover for colleagues as requested.

Plan evidence‑based health promotion programmes, order resources, compile teaching plans, and evaluate sessions.

Promote community participation and work within a community development/capacity building framework.

Act as a health link for other agencies and work closely with partners to promote the health and wellbeing of children, young people and families.

Use reflective, evidence‑based practice to influence strategy and policy development.

Be competent as a non‑medical prescriber, maintaining NMC registration and competencies when holding qualification.

Support other staff within the locality in a special interest role.

Link practice with local and national targets, delivering public health programmes based on current priorities and targets.

Contribute to the development of local policies and procedures relevant to the area of work.

Take responsibility for own clinical practice, including clinical supervision, and adhere to the NMC code of conduct.

Actively participate in reflective practice development through annual clinical observations.

Maintain live mentor status and contribute to a learning environment for SCPHNs, return to practice SCPHNs, new staff members and pre‑registration students.


Communication

Communicate complex and sensitive information concerning children or young peoples' conditions to parents/carers and young people.

Communicate effectively within multidisciplinary teams and model effective team working.

Develop links with child centres, schools, and other agencies involved in promoting children's wellbeing.

Work closely with other professional teams and key stakeholders including service users, statutory and non‑statutory organisations.

Act as first‑line response to complaints, working within national and corporate guidelines.

Contribute to the training of students of nursing and other disciplines during community experience programmes.


Budgetary responsibility

Maintain stock control, demonstrate responsible use of resources, ensure health monitoring equipment is operating to a high standard, and minimise incurred expenditure.


People management

Ensure appropriate delegation, allocation, and supervision of work and packages of care to the skill mix team; include caseload support, monitoring and counter‑signing records upon completion.

Delegate work to team members based on capabilities and competencies, ensuring clarity regarding feedback mechanisms.

Maintain own professional development and participate in appraisal and KSF process.


Research & development activity

Contribute to clinical audit and research highlighting unmet needs within the practice population.

Contribute to the maintenance of the Health Profile of the practice population.


Clinical and practice governance

Observe and maintain strict confidentiality in line with the Data Protection Act.

Adhere to Trust information governance and information security policies for all data handling.

Adopt Trust risk assessment, risk management, infection control and safeguard audit requirements.

Maintain NMC registration and complete mandatory training.

Participate in relevant emergency preparedness processes.

Write concise, accurate, timely records in line with Trust record‑keeping policy.

Participate in clinical pathway developments and service improvements.


Person specification


Qualifications and training

* Community Specialist Practitioner in Public Health Nursing (SCPHN) – Health Visitor at Honours Degree, Diploma or Masters Level
* Registered Nurse with UK NMC (RGN/RMN/RSCN/RN‑LD)
* Mentor/998 or equivalent teaching qualification (course within 1 year of commencement if no qualification)
* Registered midwife
* Management and leadership qualification


Experience

* Working with people within clinic/home and community settings
* Partnership and inter‑agency working
* Experience with children and their families
* Experience of working in the community
* Experience of Safeguarding
* Leadership and management of staff
* Experience of Mentoring
* Work with different communities including minority and ethnic groups
* Working with groups


Knowledge and skills

* High level of clinical skills, with the ability to identify, respond to and evaluate health needs
* Knowledge and effective skills in using evidence‑based models of intervention and ability to bring research into practice
* Current knowledge of local and national policies informing Health Visiting practice
* Ability to communicate effectively with children, parents, early years settings and other health organisations
* Ability to work flexibly as part of a team
* Current driving licence
* Initiating and managing change
