We are looking to recruit a motivated individual who possesses excellent communication skills, and can work well under pressure to co-ordinate discharges with families and the medical team. Main duties of the job
* Gather and be aware of information given and decisions made by the multi-disciplinary team
* Clearly present the patient's point of view to others, including promoting and protecting the interests and dignity of patients
* Communicate sensitively, confidentially and with empathy to meet the wide ranging physical and emotional needs of patients and their carers and families
* Communicate with patients, carers, relatives and professionals at all levels involved in the discharge planning process
* Communicate with Local Authority care managers to ensure consistent approach and joint working in the interests of patients
* Provide accurate, timely and relevant handover information using electronic systems where available
* Record information about patients in a safe, accurate and consistent manner using appropriate tools and techniques, including electronic and written formats as close to point of care as possible
* Maintain confidentiality at all times, as required by legislation and trust policy

Training and Qualifications
* Literacy level 2 or equivalent (GCSE A-C)
* NVQ level 3 or above in Health and Social Care or working towards an award.
Experience and knowledge
* Good reasoning, analysing and decision making skills using a variety of resources
* Experience of working within the NHS
Skills and ability
* Skilled at working with different stakeholders including patients, staff managers and clinicians

* Ability to work well in a team
