Overview
Dorset HealthCare are seeking a Community Staff Nurse to join our Integrated Care Team in Mid Dorset, within the District and Community Nurse team in Dorchester. This is a permanent position. You would work alongside our experienced Community Nurse Teams to provide a high-quality standard of care for patients in their own homes. We can be flexible with shift patterns within 08:00–20:00 to meet service needs, with some attractive weekend enhanced pay. We offer a structured induction programme, flexible working patterns and pastoral support, with excellent learning and development opportunities, including links with local universities and a preceptorship programme for newly qualified nurses.

Responsibilities
Main duties are to provide a high-quality person-centred approach to care delivery, ensuring safety, privacy and dignity while promoting the Trust values. Undertake assessment of patients’ care needs and develop care programmes as part of the wider multi-disciplinary care team.
Manage acute and chronic nursing needs, build therapeutic relationships with patients, and work closely with community partners to achieve optimal health outcomes. Identify care needs, monitor patients, record observations and help develop wellbeing care plans. Support individuals to maintain wellbeing and independence, help avoid unnecessary hospital admissions, and deliver high-quality end-of-life care including symptom management and aligning care with patient preferences.
Employment requires an Enhanced DBS check (cost covered by the Trust). Applicants subscribing to the DBS update service may present a valid certificate instead of a new check.
Qualifications & Experience
Knowledge, Skills and Training (Essential):
* Registered practitioner to degree/diploma level
* Membership of the relevant Professional Body
* Evidence of recent professional development in an up-to-date portfolio
* Knowledge and understanding of Trust strategy and policies relevant to the role
Desirable:
* Knowledge of synergy with Trust vision, values and behaviours
Experience (Essential):
* Supervised experience gained during clinical placements
* Ability to contribute to student development / supervise junior staff
* Knowledge of assessment and therapeutic interventions in the area of practice
* Experience contributing to change management
Desirable:
* Recent experience in a comparable role
* Student assessor
Management Experience (Essential):
* Experience supervising junior staff
* Ability to delegate care safely
* Prepared to act up and/or co-ordinate shifts in the absence of the team lead
Desirable:
* Clinical supervision and mentoring of junior staff
Information Technology (Essential):
* Ability to evaluate data and produce reports using Microsoft Excel and Word
* Experience using electronic patient/service user record systems
Desirable:
* Comfortable working in an agile environment
