An exciting opportunity has arisen within Hepatology at Mid Yorkshire Teaching NHS Trust for an enthusiastic, forward-thinking, and motivated nurse to join this busy service.

The post holder will have the opportunity to gain expert experience and enhance knowledge and skills within the hepatology specialty, as well as develop key advanced CNS skills in an environment supported by an experienced nursing and medical team.

The post holder will work in collaboration with other members of the multidisciplinary team to assist and coordinate a smooth pathway of care throughout the hepatology service, acting as a resource to patients and their carers.

The post holder will be able to work on their own initiative, work as a team player, and autonomously manage a caseload of patients, acting as a key worker. This role requires excellent communication skills, with the ability to coordinate care across different disciplines and organizations.


Main duties of the job

Work as an independent non-medical prescriber within the area of practice for the CNS role, taking responsibility for prescribing a range of drugs within the area of competence in accordance with MYHT medicines management policy and BNF.

Complete annual declaration of intention to practice non-medical prescribing in accordance with MYHT non-medical prescribing governance framework.

Coordinate and manage nurse-led clinics within Hepatology services.

Liaise and in-reach into inpatient wards regarding Hepatology patients, providing treatment, advice, support, and training.

Work towards Improving Quality in Liver Services (IQILS) within MYHT.

Assist in the development of a Paracentesis Service within MYHT.

Coordinate patient care through advice line for hepatology patients.

Job responsibilities

Key requirements

* Responsibility for a defined caseload of patients, providing an effective and high-quality patient-focused service sensitive to the needs of users and providers.
* Organize, prioritize, and evaluate workload based on accepted referral and discharge criteria in line with operational protocols.
* Provide clinical, social, and psychological support using advanced communication skills, education, and health promotion to enable patients to reach an optimum level of independence and health.
* Responsibility for delivering specialist nursing skills within complex pathways, providing advice and support to a defined caseload and other healthcare providers.
* Ensure evidence-based practice.
* Lead aspects of the service development, including care systems, protocols, and guidelines, working as part of a multidisciplinary team and fostering interdepartmental relationships focused on quality improvement.
* Ensure cost-effective use of resources and appropriate governance management.
* Lead health education and health promotion programs relevant to hepatology.
* Work within national policy frameworks and local trust guidance.
* Review and interpret diagnostic investigations and point-of-care testing, adjusting care plans accordingly in consultation with medical staff.
* Handle patient/service user issues professionally, collaborating with PALS as needed, and proactively resolve issues and learn from complaints.
* Maintain accurate and concise patient records within agreed databases and electronic systems.
* Contribute to and lead multidisciplinary management of patients and carers, ensuring ongoing care needs are met.
* Act as a resource for clinical staff on wards/departments regarding specialist knowledge.
* Ensure effective multidisciplinary communication.
* Provide mentorship and preceptorship to staff and students as required.
* Produce annual service reports and other reports as needed.
* Work within an agreed divisional work plan.
* Manage staffing capacity and service needs for effective clinical coverage.
