We are seeking an enthusiastic Clinical Independent Prescribing Pharmacist to join our established team and be a part of a company that really makes a difference.
As a GP Practice Pharmacist, you will work within a GP practice providing patient-facing dispensing, advice, treatment and administrative support.
Our Clinical pharmacists work as part of the general practice team and are supported by, and work with, a diverse multi-skilled professional team of clinical and non-clinical friendly staff, providing high quality, person‑centred, holistic care to patients registered with Omnes Healthcare practices.
Main duties of the job
* Conducting patient consultations and clinical assessments.
* Prescribing and reviewing medicines.
* Managing long‑term conditions such as diabetes, asthma, COPD, and hypertension.
* Carrying out structured medication reviews.
* Monitoring high‑risk medicines and ensuring safe prescribing.
* Responding to acute medication requests.
* Reconciling medicines following hospital discharge.
* Providing medicines advice to patients and clinicians.
* Auditing prescribing practices and supporting medicines optimisation.
