Social Worker required for work within BANES in Hospital Discharge Team. 37 hours a week with rates up to £32.50-38ph for experienced candidates. Contract will be minimum 3 months. Flexible, hybrid working - 2 days in the office.
Bath & North East Somerset Council is seeking an experienced Social Worker to join their Hospital Discharge Team. This is a fast-paced and rewarding role focused on supporting safe and timely discharges from hospital, ensuring individuals receive the right care and support in the community.
Key Responsibilities:
Support hospital discharges in line with discharge to assess pathways.
Complete Care Act assessments and develop person-centred care plans.
Work collaboratively with health professionals, families, and community services.
Undertake safeguarding enquiries where required.
Promote independence and ensure positive outcomes for service users.
Manage a caseload effectively within a busy hospital setting.
You will be SWE registered, ideally a driver with access to own car and able to reside and work in the UK
