Offer description

Overview

This is a brand new post providing Pharmacy expertise and developing the role of community based pharmacists working in the Weymouth and Portland Community Mental Health Team. The role supports the NHS 10 year plan, working with new integrated care models to provide a significant patient facing role and medication related support and advice to HCPs and the Weymouth Community. The successful candidate should be a registered independent prescriber. Applications from candidates that require current Skilled worker sponsorship to work in the UK are unfortunately ineligible to apply for this position. Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

* To lead, deliver, develop and evaluate specialist clinical mental health pharmacy services in Weymouth and Portland.
* Work as an advanced pharmacist as part of a multi-disciplinary team, to provide highly specialised advice in medicines optimisation across designated CMHTs and associated PCNs and neighbourhood teams.
* To support and influence PCN Pharmacists and GP-based Clinical Pharmacists to ensure the pharmaceutical needs of patients with mental illness are met through partnership working at the interface.
* Communicate highly complex clinical pharmacy advice on the management of patients with mental illness to maximise benefit and minimise risk in relation to medicines.
