About the Role:

The D2A (Discharge to Assess) service provides short-term, high-quality support to individuals who are well enough not to need an acute hospital bed but do need some extra support in their home. You will ....
* Write up support plans following the initial visit, ensuring person-centred care is provided
* Motivate customers, building their confidence so they can achieve their goals and return to independent living
* Support customers with tasks around the home and with personal care
* Work a mixture of early and late shifts between 7 am and 10 pm and alternate weekends.
* Be on-call on a roster basis which attracts an additional payment.
About you:
* You will hold (or be working towards) a QCF Level 3 in Health and Social Care.
* Previous experience in caring for vulnerable adults with a wide range of complex needs is essential.
* Sensitive, caring, and passionate about delivering a first-class service to people within the local community.
* A full driving licence and access to your own vehicle are required as you will need to be comfortable with lone working and travelling between customers.
* You will support clients across the whole of Sedgemoor, including Burnham, Cheddar and Bridgwater. You are paid per shift meaning your hands on care and travel time is all included within a shift.
Role Benefits:
* £500 welcome bonus on successful completion of your probationary period.
* Double time on bank holidays and an extra £1 per hour on weekends.
* Mileage allowance of 45p per mile
* Equivalent to 30 days of paid annual leave (inclusive of bank holidays), increasing to the equivalent of 33 after 5 years' service (pro-rata)

About the Team:

Our specialist D2A (Discharge to Assess) team covers the areas of Bridgwater, Highbridge, Burnham-On-Sea, Cheddar and surrounding areas. The team works with clients who have been discharged from hospital, supporting them to recover in the place they call home.
