Responsibilities/What you'll Do:
* Create and deliver a varied, personalised activities programme from one-to-one sessions to group events tailored to each residents individual interests, abilities and life history
* Plan and organise community outings and trips, arranging appropriate transport
* Build relationships with local community groups, schools, volunteers and performers to bring fresh experiences into the home
* Keep meaningful records of resident engagement to help the wider care team understand each person better
* Support and inspire carers to weave activity into everyday care moments
* Champion residents voices, making sure their preferences always shape the programme
* Plan and maintain an activities programme that sustains interest and progression over time
