Emergency medicine clinical fellow portfolio programme

Worcester
Worcestershire Acute Hospitals NHS Trust
Posted: 16 February
Offer description

Job summary

The Portfolio Pathway (formerly known as CESR) is the route to specialist registration for doctors who have not completed a GMC-approved programme in the relevant specialty, but who can demonstrate that they have the relevant knowledge, skills and experience. This post would therefore be suitable for Resident Doctors looking to pursue a portfolio route as an alternative to formal specialty training.

Depending on the years of experience in Emergency Medicine, NHS experience and proven competencies, the successful candidate would start on Tier 2, Tier 3 or Tier 4 rota (dependent on previous experience).

We initially offer a 1-year contract. Following this period, your contract will be subject to an annual review based on the achievement of specified criteria, including a successful Educational Supervisor and ARCP Faculty Report evidencing completion of WBPAs and other requirements set out at the start of your year. If the review is successful, your contract will be extended for a further 12 months. These extensions will continue annually, supporting your progression through the 4-year programme, and will enable you to complete the Pathway portfolio and secondments. Please note that there is no guarantee of a 4-year contract from the outset.

Placements will be across both the Alexandra Emergency Department and the Worcester Royal Emergency Department. There are RCEM Portfolio Programme (CESR) Supervisors and Assessors at both sites.

Main duties of the job

Participating fully in all the educational activities of the department

Assisting the Consultants in the day-to-day clinical care of the ED patients in all areas of the department including Resus, Majors, Minors, Paediatrics and Ambulatory care.

Participating as a team member in resuscitation teams under supervision

Participating in the Clinical Audit programme and the quality control of the department

Support the Trust in the delivery of a high standard of care and meeting Trust and departmental objectives by timely service and quality led clinical care.

Comply with relevant trust policies including those relating to data protection

Actively pursue professional development maintaining a portfolio of evidence so that you are able to participate satisfactorily the appraisal process in accordance with GMC requirements.

Worcester Acute PORTFOLIO PATHWAY

Our commitment to you

The WAHT Portfolio Pathway faculty are committed to the progression of the Portfolio Pathway trainee. We understand that people choose this pathway due to its flexibility compared to formal deanery training, therefore we work with the trainee to create an individualised programme; and to provide you with the support you to achieve your goals through:

Educational Supervision

The Portfolio Pathway trainee will be assigned a RCEM approved Portfolio Pathway Clinical Supervisor at the start of the programme. The Clinical Supervisor will help identify any specific educational needs, design a personal development plan and provide some guidance on career progression. There will be an Annual Review of Competency Progression (ARCP), which will include the whole Consultant team.

Curriculum coverage

We are committed to ensure that you are able to access all areas of the curriculum through clinical experience.

At the end of the programme, we will support the trainee with submission of their portfolio to the GMC for consideration for entry to the Emergency Medicine Specialist Register (to become a Consultant).

Your commitment to the Portfolio Pathway Programme will be evidenced by

Attending regular Clinical Supervisor meetings

Attendance at departmental teaching

Enrolment and engagement with RCEM Kaizen ePortfolio

Work place based assessments (WPBA)

Reflective learning evidenced in the ePortfolio

Maintaining a procedural log-book

Attending a POCUS course

Engagement with the examinations process

Involvement with Audit/Quality Improvement Projects/Research

Successful Annual Review Competency Progression (ARCP)

Secondments

Secondment placements will only be available to those who have successfully completed the first 12 months of the program and evidenced satisfactory progression.

We have created a Portfolio Pathway Programme that is 4 years (WTE) duration following the initial 12 month period of working in one of the WAHT Emergency Departments. The programme will support all of the secondments that are required for the Emergency Medicine Portfolio Pathway.

The secondments will be tailored to the needs of the doctor but generally would appear as below for a doctor who is full time:

Year 1: 12 months Emergency Medicine (unless 12 months in EM at WAHT has already been completed)

Year 2: 9 months Emergency Medicine and 3 months Paediatric Emergency Medicine (WRH)

Year 3: 9 months Emergency Medicine and 3 months Anaesthetics (Initial Anaesthetic Competencies IAC) (WRH/AGH)

Year 4: 9 months Emergency Medicine and 3 months Intensive Care Medicine (WRH/AGH)

Year 5: 9 months Emergency Medicine and 3 months Acute Medicine (WRH/AGH)

For those based at the Alexandra Hospital Emergency Department, we will also offer a Trauma placement at WRH to help with the Major Trauma aspects of the curriculum.
