This is a specialist role where you will manage a complex, high-risk caseload, applying your expertise to assess needs, design person-centered care plans, and deliver empowering outcomes. You will champion a reablement approach, helping individuals reach their full potential while ensuring safety, dignity, and independence.
With significant autonomy, you will use professional judgement and innovative, solution-focused thinking to shape services and improve lives across Cumbria. Collaboration is key, you will work closely with health partners, community organisations, and other agencies, while also providing case supervision and professional guidance to colleagues.

This role requires a DBS check. The level of check for this role is Enhanced DBS Check with an Adults Barred List check. Please note this role involves working in regulated activity with adults.
