Job summary
We have an exciting opportunity for an experienced Extended Clinical Practitioner to join our forward thinking and innovative GP groups at either The Willow Group or Shakespeare Road.
* The Willow Group provides services for 49,000 patients over 5 local sites in Gosport.
* Shakespeare Road provides services for 13,000 patients in Basingstoke.
We are looking for an enthusiastic, caring and motivated professional with excellent clinical, organisational & communication skills, who has the ability to work autonomously.
The post holder will be an experienced nurse/AHP, acting within their professional boundaries, to support our on-going initiatives. The post holder will also hold their own clinics and demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the general practice team and extended primary care team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership. They will look to develop new and innovative concepts, models, methods and practices to deliver new and improved primary care services to meet the needs of the practice population.
Main duties, tasks & skills required
The post holder is an experienced nurse who aspires to provide a responsive, safe, effective and caring service to patients requires initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinic decision making and expert care, including assessment and diagnostic skills, for patients within the general practice.
Along with using a medical model to assess, diagnose and prescribe for acute medical conditions, the clinician will use a holistic approach to consider methods and interventions to prevent recurrence of problems and complete referrals to appropriate members of the wider multidisciplinary team.
The clinician will participate in, and influence efforts, across health and social services to shape multi-disciplinary pathways designed to support patient choice, improve quality of life, promote self-management and assure early intervention through proactive provision of care in or as close to the patients' own home as possible.
The role will include the provision of strong leadership to the team, including day to day leadership responsibilities, development and creative leadership, responsive leadership, enabling the team to achieve a 'well led' culture.

Qualifications
Essential
* Appropriate professional qualification (nursing or allied health professional) that allows registration with the NMC or HCPC
* Professional UK registration e.g. NMC / HCPC
* Degree level award
* Independent non-medical prescribing or pharmacology level 7 profession relevant qualification.
* Holds level 7 Clinical History Taking & Physical Examination
* Holds level 7 Diagnostics and Decision making
* Able to demonstrate a good command of the English Language both written and verbal
* Computer Literate in both data entry and communicating via IT - emails and report
