 
        
        Job Overview
The post holder is a Mental Health & Wellbeing Coach, who works alongside the general practice team and a mental health multi-disciplinary team, providing non‑clinical coaching and wellbeing support only within NN4 PCN. The individual will act within their professional boundaries to work for the benefit of patients, providing coaching support to improve wellbeing without providing clinical treatment. They will provide assistance and advice to other members of the primary healthcare team in managing the mental wellbeing of the practice population.
Responsibilities
 * Participate in regular individual and group coaching‑focussed supervision with a suitably qualified supervisor.
 * Develop collaborative relationships and work in partnership with health, social care, community, and voluntary sector providers, as well as multidisciplinary teams, to support patients wider health and wellbeing and contribute to reducing health inequalities.
 * Provide education and specialist expertise to PCN staff, supporting them to improve their skills and understanding of personalised care and behavioural approaches, and ensuring consistency in the follow‑up of patients’ goals with MDT input.
 * Raise awareness within the PCN of shared‑decision making and decision support tools.
 * Demonstrate understanding of safeguarding processes for vulnerable people.
 * Implement basic safeguarding processes for vulnerable individuals.
 * Support and strengthen primary care and promote practices to improve quality by working with and on behalf of primary care in Norfolk.
 * Help with challenges identified by practice workload, GP recruitment, and sharing best practice.
 * Recognise the changing nature of primary care and assist to ensure that the right level of service is provided to address the needs of the local population.
 * Provide benefit to patients of primary care, clinicians and administrators who work in it, and practice organisations in Norfolk.
 * Work within health systems to provide patient care in innovative and solution‑focussed ways.
Remuneration
Salary: Up to £23,900.27
Place of work: Practices in NN4, a PCN in North Norfolk - Acle Medical Partnership, Brundall Medical Partnership, Biofield Surgery, Hoveton & Wroxham Medical Centre, Staithe Surgery, Ludham & Stalham Green Surgery.
Hours: 28 hours per week
