Main duties of the job
* To work as part of the trust wide Intermediate Care Rehabilitation Service .
* To work as part of a 7-day service over weekends and Bank Holidays on a rota basis
* To hold a case load of non-complex cases under the supervision of a senior team member
* To undertake specific support work to discharge processes from the intermediate care units.
* To work unsupervised, reporting back on patient progress and escalation of any problems.
* To assist in the induction of new staff within the ward area, supporting knowledge
Qualifications
* GCSE grade 4 or GCE C and above
* Level 2 functional skills or health care support worker apprenticeship / NVQ level 2 in relevant subject
* Level 3 apprenticeship for example senior health care support worker or similar and / or NVQ level 3 in relevant subject or equivalent level of knowledge and experience
Experience
* Experience in health or social care settings such as employment, work experience volunteering or placements
* Demonstrate the values of the NHS and commitment to personal and professional development
