The Role

NHS Lanarkshire has an exciting opportunity for a pharmacist looking to enhance their clinical skills and increase job satisfaction in a patient centred care home role. We are looking for an enthusiastic and proactive individual to join our team in shaping the pharmacy input to care homes. Our vision for the pharmacy care home service is to ensure all care home residents across Lanarkshire get the best from their medicines by taking a realistic medicine approach to the care we provide always with what matters to the patient in mind. This role is an excellent developmental opportunity to give pharmacists a breadth of experience across the diverse, interesting & engaging clinical nature of primary care focussing on care home residents.

This role will support the development and implementation of the pharmacy care home service whilst also delivering it in the form of medication reviews (prescribing where necessary) with care home patients across Lanarkshire. They will form part of a growing MDT Care Home team carrying out medication reviews as an independent prescriber, with more complex cases that require input from a wider team being presented at a weekly MDT review which is supported by care of the elderly consultant.

You will work closely with GPs, consultants, other practice staff, care home staff, care home liaison nurse team, members of the primary healthcare team and central team colleagues in supporting the development and application of high quality, safe, cost effective, rational and evidence based prescribing. You will work within GP practices, care homes and other healthcare settings to provide individualised clinical care to patients to improve the safety and effectiveness of their medicines, prescribing where appropriate. You should have a sound clinical knowledge, good IT skills, excellent communication and interpersonal skills and be able to work autonomously as well as part of a team.

This is an exciting opportunity for a 6 months maternity cover and due to the need to fill the position quickly interviews are likely to be held on Thursday 2nd April 2026.

In this key role, you will:

1. The function of the Prescribing Management Team is to promote safe, effective and economic use of medicines, and to facilitate integrated patient-focused care that meets the present and anticipated needs of the population of NHS Lanarkshire, in accordance with local and national strategies.
2. The post-holders role is integral to delivery of the pharmacotherapy service as defined in the General Medicine Services (GMS) Contract (GMS 2018) and the delivery of the Prescribing Quality and Efficiency (PQE) Strategy.
3. This post holder will have a varied role including GP practice based commitments, working within a hub setting and undertaking PQE including polypharmacy reviews. There will be the opportunities to progress their leadership and management experience to align with advanced practice but mentoring and supervising foundation pharmacists.

What You'll Bring

4. Masters of Pharmacy Degree
5. Registered with the General Pharmaceutical Council (1-year pre-registration practice and competency assessment with examination to enter the register)
6. Required to demonstrate a highly specialised clinical pharmacy knowledge, skills clinical reasoning and judgement
7. Experience in patient information/education
8. A driving licence is required, this must be a full UK/EU/EEA licence.

From 1 April 2026, the Agenda for Change full-time working week will reduce from 37 hours to 36 hours. Part-time working hours will be reduced on a pro-rata basis. A corresponding increase in the hourly rate will apply, ensuring that overall pay remains unchanged.
