The Older Adult Mental Health Service has worked with Walsall CCG in enhancing existing Community Mental Health Services creating an Enhanced Community Mental Health Team for Older Adults from 09:00hrs to 17:00hrs, 7 days a week. An exciting opportunity has arisen within our team and we are currently looking to recruit an experienced RMN to become a valued member of our team. You will be responsible for assessment, care planning, risk assessment and risk management with a holistic person centred approach towards care for Older Adults over the age of 65 presenting with either Functional Mental Health needs and or Dementia. This will be achieved via carrying a manageable caseload with wider MDT support.
We are an energetic, hardworking and successful team with good patient outcomes. The successful applicant will need to be an enthusiastic and committed individual and will be part of a fully integrated Multi-Disciplinary Team comprising of CPNs, Occupational Therapists, Support Workers, Nurse Practitioners, a Clinical Lead, Team Manager, Clinical Psychologist and Psychiatric Medical Teams.
Main duties of the job
To practice within the NMC code of professional conduct.
To assess the care needs, development, implementation and evaluation of programmes of care in the context of the care programme approach.
The post holder will be responsible for their own area of work and ensuring adequate daily contacts to meet both individual and team targets.
The enhanced ECMHTOA is central to the efficient running of the service with responsibility for gate keeping the inpatient beds.
To take responsibility for service users who are in crisis and to remain involved until the crisis is over.
The post holder will work with core and associate members of ECMHTOA and accept referrals from sources such as GPs, Acute Inpatient Services, Home Treatment and Mental Health Liaison based at General Hospitals.
To take overall clinical responsibility for service users on the ECMHTOA caseload.
To develop an interface with Inpatient Services
To undertake risk assessments on a regular basis and formulate risk management plans.
To participate in multi-disciplinary review meetings.
Ensure an understanding of embedding of Trust Visions and Values in everyday practice.
