Key Responsibilities
 * Conduct comprehensive clinical assessments and formulate management plans.
 * Diagnose and manage acute and chronic presentations.
 * Provide expert care for frail and elderly patients, including care home visits.
 * Prescribe independently (V300 qualification essential).
 * Order and interpret diagnostic tests and investigations.
 * Work collaboratively with GPs, nurses, pharmacists, and allied health professionals.
 * Contribute to service development, audits, and clinical governance activities.
 * Support colleagues and learners through mentorship and professional guidance.

Qualifications
 * Recent experience in a Primary Care setting
 * Non-medical prescriber
 * BSC/MSc Advanced practice or equivalent
 * NMC / HCPC registered
 * UK based (necessity)
 * Excellent written and verbal communication skills
 * A passion for healthcare
 * Motivated and committed to developing the knowledge and skills required for the role and for ongoing professional development
 * Registered Nurse/paramedic (essential)
 * Full UK driving licence
Desirable Qualifications
 * Experience in long-term condition (LTC) management (e.g. COPD, diabetes, heart failure)
 * Leadership or teaching experience
 * Proven experience in the assessment and management of frail adults and/or care home patients
