Job Title: Band 6 CPN – Community Psychiatric Nurse Location: Grand Union Village, West London NHS Trust Rate: £27 per hour Ltd (PAYE options available) Contract Type: Long-term NHS contract Hours: Monday to Friday, Full-time (37.5 hours per week) Job Description: We are currently seeking an experienced Band 6 Community Psychiatric Nurse (CPN) to join a well-established NHS community mental health team at Grand Union Village, part of West London NHS Trust. This is an excellent opportunity for a CPN looking for a long-term assignment within a supportive and multidisciplinary environment. You will be working with adults with a range of mental health conditions, contributing to recovery-focused care plans, and managing a defined caseload under the CPA framework. Key Responsibilities: Manage and coordinate care for a caseload of service users with complex mental health needs. Conduct risk assessments, mental state examinations, and reviews under the CPA process. Deliver interventions including psychoeducation, relapse prevention, and medication management. Work in collaboration with psychiatrists, psychologists, social workers, and support staff. Provide crisis intervention and facilitate referrals to specialist services where required. Maintain accurate and timely clinical records using NHS systems (e.g., RiO). Requirements: Registered Mental Health Nurse (RMN) with current NMC registration. Minimum 2 years post-registration experience in community mental health. Experience working at Band 6 level or equivalent in an NHS or community setting. Strong assessment, care planning, and risk management skills. Excellent communication and interpersonal abilities. Able to work autonomously and as part of a multidisciplinary team. Up-to-date PMVA and mandatory training desirable.
