Job summary

Band 6 Occupational Therapist - Discharge to AssessSecondment / Fixed Term Contract considered alsoTaunton and West Somerset/ Sedgemoor AreasWe are looking for a dynamic and inspiring Occupational Therapist to join the Discharge to Assess (D2A) service part of the wider Intermediate Care service for Somerset.You will gain an in depth understanding of community working and supporting people within their own homes.Successful candidates will be directly supported by our Senior Occupational Therapists and Advanced Practitioner/Clinical Specialist within the team.A vast number of people have been helped to regain their independence at home following a stay in hospital thanks to Somerset's Discharge to Assess service. "It means our patients can return home much quicker than before, reducing the need to wait in hospital for a long-term care package."


Main duties of the job

You will be expected to carry out specific therapy assessments which will include goal setting and treatment planning, assessing risk, promoting and enabling independent functioning. You will be committed to a person-centred approach to your work, have an ability to prioritise and work flexibly. * Advanced clinical reasoning to enable sound judgement and intervention when treating patients
* Ability to work autonomously and set priorities


Desirable

* Post registration experience as an Occupational Therapist including experience in a ward setting to include treatment of those with physical and mental health conditions


Qualifications

* Diploma/Degree in Occupational Therapy (OT)
* Registration with Health Care Professions Council
* Knowledge of community Occupational Therapy role
* Car driver


Desirable

* The post holder must have excellent verbal and written communication skills to deal with the complex needs of these patients, carers and relatives, and maybe exposed to situations and environments that may cause distress.
* To be able to use advanced clinical reasoning to predict achievable outcomes of treatment and to recommend the best course of intervention, developing comprehensive patient cantered treatment plans, in conjunction with other professions and agencies.
